Take Control of Your Diabetes with Core Health Partners' Self-Management Program
Diabetes is one of the biggest global health crises of our time. Over 34 million Americans are living with diabetes, and another 88 million have pre-diabetes, a condition that, if left unaddressed, can lead to type 2 diabetes and related health complications like heart disease, nerve damage, and kidney failure. Diabetes is a chronic condition, but it can be managed with proper self-care, education, and support. At the Togetherhood Initiative Center, in collaboration with Core Health Partners, delivers Diabetes Self-management Education Support (DSMES) program to help you live a healthier, happier life with diabetes.
What is Diabetes Self-Management?
Diabetes self-management is the process of actively managing your diabetes daily. This includes monitoring your blood sugar levels, taking medication as prescribed, making healthy food choices, getting regular physical activity, and working closely with your healthcare team. Diabetes self-management also involves knowledge and skills related to problem-solving, goal setting, stress management, and coping with the emotional impact of diabetes. By taking an active role in your diabetes care, you can improve your blood sugar control, prevent, or delay complications, and enjoy a better quality of life.
Research shows that diabetes self-management can lower blood sugar levels, reduce the risk of diabetes-related complications, and improve overall health outcomes. By taking control of your diabetes, you can also save money on medical bills by preventing hospitalizations, emergency room visits, and costly procedures. Diabetes self-management can also help you feel better physically and emotionally, as you learn to manage the daily challenges of living with diabetes.
At the Togetherhood Initiative Center in Immokalee, we offer a comprehensive Diabetes Self-Management and Education Support program to help you get on the pathway to wellness. Trained healthcare professionals from Core Health Partners will work with you to develop a personalized diabetes care plan, including individualized education and support to manage your diabetes effectively. Our program includes one-on-one consultations, group sessions, and ongoing resources to help you stay motivated and informed about your condition.
How Can You Get Started?
Getting started with our Diabetes Self-Management and Education program is easy. You can self-refer by filling out the form on our website (use button below) or by calling our Immokalee office to schedule an appointment. Our program is covered by most insurances, Medicaid, and Medicare, and we also offer a sliding fee discount for eligible patients. By taking the first step towards self-management, you can take control of your diabetes and live a healthier, happier life.
